Community-Based Adult Services

 

We serve older adults and persons with disabilities who need a higher level of care, but can still benefit from receiving services in a group setting. Our program offers the following, but is not limited to:

Physical, Occupational, Speech Therapy 

Nursing management and treatment 

Assistance with personal care needs such as toileting, dressing, diaper changing, oral hygiene, and grooming

Dietary services and g-tube feeding 

Behavior Management 

Social Services

Individualized Plan of Care

Transportation

We also provide at least one meal and a snack each day. Food menus are changed on a monthly basis under the approval of a Registered Dietitian. Therapeutic diets are also prepared and provided based on primary care physician's orders of each participant.
